Identity Theft
You should file a report with your local police precinct for identity theft so that the matter can be investigated and documented. Ask for a copy of the police report in case you are asked to supply such by bank, credit card company, or elsewhere. You might ask the credit reporting agencies if a copy of the report, or at least its file number, could be made part of the agency's records.
